Our Vision

Change Tracker Minimize

Change Tracker is a tool for practitioners to use within their own settings and services in South Gloucestershire.  It aims to support planning to improve outcomes for children and young people, and also to measure the progess tha each plan receives. 

It is hoped that children, young people and their families will be involved in using Change Tracker as a means of engaging them in being a part of every support plan and also to support them on objectively defining their own needs and desired outcomes.

The South Gloucestershire Partnership for Children and Young People will be exploring ways to use the tool to identify and measure the impact of a variety of interventions across the county. First Point Minimize

First Point is a new single point of access to children's preventative, social care and specialist services in South Gloucestershire.

One of the aims of First Point is to prevent families from having to give the same information more than once.  We have introduced a First Point Form which will continue to be developed to support this aim. There is also an Information Sharing Consent Form which must be used whenever a First Point form is completed and before a CAF assessment is started.

Key questions that were raised at a briefing session can be read here.

Tracy Allison, Head of Integrated Services for Children, Adults & Health held briefing sessions for CYP integrated services staff, practitioners/professionals and partner agencies.  View the presentation here.

Two leaflets about information sharing have been produced, one for parents and carers and one for young people.

The Care Forum Annual Report 2013 - added 18 March 2014

This report is all about how our sector has kept to its roots and developed new shoots despite the storm. We have all had to be innovative, working in new ways and forging new partnerships. It has not been an easy year and our thanks go to all our staff, volunteers and member organisations who work so hard.

Cossham Hospital - A New Base for Out of Hours GP Service - added 18 March 2014

From May 2014 Cossham Hospital will be the new base for South Gloucestershire's GPs out-of-hours' service.  The service provides telephone advice and face to face consultations with a GP or nurse outside normal opening hours for GP surgeries and includes home visits for patients who are housebound.
